Continuity of Care
After my retirement from clinical practice, I will no longer be available for consultations. I encourage you to continue your care with another urologist and your general practitioner (GP), who can provide an ongoing referral if needed. I am pleased to let you know that Dr Henry Yao (urologist) will be available to take over the care of patients from This practice. Dr. Yao has completed three years of fellowship training with the first year being with myself at Western Health, a second year in the UK in Robotic Surgery and Uro-oncology and a third year in Canada in Reconstructive and Functional Urology. He has subspecialty interest in the management of overactive bladder, urinary incontinence, urethral stricture, benign prostate enlargement, neurogenic bladder and pelvic organ prolapse and will be well placed to provide you with ongoing care.
MEDICAL RECORDS
As Dr Yao will be taking over the practice at his consulting locations upon my retirement, he will have access to your existing clinical notes and this can assist with your continuity of care. This ensures that your medical history and prior treatments are available to inform any future consultations and decisions. If, however, you would prefer to continue your care with another urologist or doctor, you are welcome to request that your file be transferred to them, or that a copy be provided directly to you. To arrange this, please complete a consent form. or requests can be made in writing to:

Your medical records will be retained for the legally required period (at least seven years, or longer if you were under 18 at the time of treatment).
