Medical Record Access

Continuity of Care is crucial for your ongoing health management. After my retirement from clinical practice, I will no longer be available for consultations. I encourage you to continue your urologist care with another qualified professional and your general practitioner (GP), who can provide an ongoing referral if needed. I am pleased to let you know that Dr. Henry Yao, a skilled urologist, will be available to take over the care of patients from this practice. Dr. Yao has completed three years of fellowship training, beginning with my mentorship at Western Health, followed by a year in the UK focusing on Robotic Surgery and Uro-oncology, and a third year in Canada specializing in Reconstructive and Functional Urology. He has a subspecialty interest in the management of overactive bladder, urinary incontinence, urethral stricture, benign prostate enlargement, neurogenic bladder, and pelvic organ prolapse. Dr. Yao will be well-positioned to ensure continuity of care and manage your patient medical records effectively.

MEDICAL RECORDS

As Dr. Yao will be taking over the practice at his consulting locations upon my retirement, he will have access to your existing patient medical records. This access will assist in ensuring continuity of care, as your medical history and prior treatments will be available to inform any future consultations and decisions. If, however, you would prefer to continue your urologist care with another doctor, you are welcome to request that your file be transferred to them, or that a copy be provided directly to you. Your medical records will be retained for the legally required period (at least seven years, or longer if you were under 18 at the time of treatment).
